Qualifications:

Education: M.D., D.O.

Board-certified or board-eligible in Family Medicine, Internal Medicine, or Pediatrics.

Completion of a Sports Medicine Fellowship, with a CAQ certification.

Strong clinical skills with a commitment to patient-centered care.

Active, unrestricted FL license (or the ability to obtain FL license) is required
