Clinical Medical Social Work- Primary Care

 

 

We are looking for passionate master’s level social workers to serve as an integrated member of a healthcare team providing accessible primary care in clinics throughout Columbus neighborhoods.

As a neighborhood partner, you will build relationships with community resources, the families served by the clinics, and the interprofessional team to support families’ own abilities to access and benefit from available services. You will be part of a 20+ primary care social work team, throughout Columbus, who work together to address the needs of families.

Provides developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license.

    • Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health (Continuously).

     

    • Clinical practice includes evidenced based screening tools, psychosocial and risk assessments, and treatment planning.

     

    • Participates within the multidisciplinary team to implement and evaluate practice based on clinical evidence and patient/family preferences to improve care.

     

    • Provides supportive counseling, brief interventions and anticipatory guidance related to emotional, behavioral, educational, psychological, or social concerns 

     

    • Engages in reflection, consultation, education, and training with teams to address psychosocial issues that impact the delivery of equitable/optimal health care services.

Education: MSW required

 

Licensure: LSW required, LISW preferred

 

Experience: 2 years clinical social work experience preferred.
